มหิดลใช้คณิตศาสตร์ช่วยผ่าตัด

มหิดลใช้คณิตศาสตร์ช่วยผ่าตัดบายพาส ลดปัญหาผู้ป่วยหลอดเลือดหัวใจอุดตันซ้ำ
ทีมแพทย์และนักวิจัยมหิดลพัฒนาแบบจำลองคณิตศาสตร์ ช่วยผ่าตัดบายพาสผู้ป่วยหลอดเลือดหัวใจอุดตัน ใช้ข้อมูลภาพซีทีสแกนสร้างหลอดเลือดจำลองเสมือนจริงได้สมบูรณ์แบบที่สุดในโลก ช่วยแพทย์เข้าใจปัญหาของโรคและเห็นการไหลของเลือด พร้อมออกแบบการทำบายพาสที่เหมาะสม เห็นผลการผ่าตัดก่อนลงมือจริง ช่วยลดปัญหาผู้ป่วยต้องเข้ารับการผ่าตัดใหม่
รศ.ดร.เบญจวรรณ วิวัฒนปฐพี อาจารย์ภาควิชาคณิตศาสตร์ คณะวิทยาศาสตร์ ร่วมกับ ผศ.นพ.ทนงชัย สิริอภิสิทธิ์ ภาควิชารังสีวิทยา คณะแพทยศาสตร์ ศิริราชพยาบาล มหาวิทยาลัยมหิดล แถลงผลสำเร็จของโครงการวิจัย "เทคนิคการจำลองแบบคณิตศาสตร์เพื่อช่วยการผ่าตัดบายพาสหลอดเลือดหัวใจ" ที่คณะวิทยาศาสตร์ มหาวิทยาลัยมหิดล (พญาไท) เมื่อต้นเดือนมิ.ย.ที่ผ่านมา
ผศ.นพ.ทนงชัย กล่าวว่า จากปัญหาการผ่าตัดบายพาสหลอดเลือดหัวใจที่ผ่านมา ของศิริราชพยาบาลที่มีผู้ป่วยเฉลี่ยปีละกว่า 1,200 ราย พบว่า 25% ของผู้ป่วยที่ได้รับการผ่าตัดบายพาส จะต้องกลับมาเข้ารับการผ่าตัดใหม่อีกครั้งภายใน 1 ปี ขณะที่ผู้ป่วยอีกกว่า 50% ต้องกลับมารับการผ่าตัดใหม่อีกครั้งภายใน 10 ปี เพราะหลอดเลือดหัวใจอุดตันซ้ำ ซึ่งคาดว่าเป็นเพราะแพทย์ยังไม่เข้าใจกลไกและปัญหาของโรค รวมถึงผลที่เกิดจากการผ่าตัดบายพาสอย่างละเอียดลึกซึ้งมากพอ จึงได้ร่วมกับ รศ.ดร.เบญจวรรณ พัฒนาแบบจำลองทางคณิตศาสตร์เพื่อศึกษาสิ่งที่เกิดขึ้นในหลอดเลือดหัวใจทั้งก่อนและหลังการผ่าตัด เพื่อช่วยในการวางแผนการรักษาได้อย่างมีประสิทธิภาพและประหยัดเวลาก่อนลงมือผ่าตัดจริง
รศ.ดร.เบญจวรรณ อธิบายกับทีมข่าววิทยาศาสตร์ ASTVผู้จัดการออนไลน์ว่า ได้นำข้อมูลจริงจากผู้ป่วยโรคหลอดเลือดหัวใจจำนวน 50 ราย มาพัฒนาแบบจำลองทางคณิตศาสตร์ และใช้ข้อมูลภาพซีทีสแกน (CT scan) มาสร้างแบบจำลองหลอดเลือดหัวใจเสมือนจริงสามมิติ และใช้เทคนิคพิเศษในการปรับแต่งผิวหลอดเลือดหัวใจให้เรียบ ทำให้ได้ภาพสามมิติเสมือนจริงของหลอดเลือดหัวใจที่มีแขนงหลอดเลือดสมบูรณ์ที่สุดในขณะนี้ เพื่อใช้อธิบายการไหลของเลือด แรงดันเลือด และแรงเค้นที่หลอดเลือด ได้อย่างเข้าใจและน่าเชื่อถือยิ่งขึ้น
"แบบจำลองคณิตศาสตร์ที่พัฒนาขึ้น จะช่วยอธิบายกลไกการเกิดโรคหลอดเลือดหัวใจในผู้ป่วยแต่ละรายได้ โดยแพทย์จะต้องวัดแรงดันและอัตราการไหลของเลือดในหลอดเลือดหัวใจมาใช้คำนวณในแบบจำลอง และนำภาพซีทีสแกนหลอดเลือดหัวใจมาสร้างแบบจำลองสามมิติ เพื่อศึกษากลไกต่างๆ ซึ่งจะทำให้แพทย์เข้าใจการไหลของเลือด แรงดันเลือด แรงเค้นที่ผนังหลอดเลือดของผู้ป่วยได้ดียิ่งขึ้น แล้ววางแผนว่าควรจะทำการผ่าตัดบายพาสอย่างไรจึงจะเหมาะสมที่สุดในผู้ป่วยแต่ละราย" รศ.ดร.เบญจวรรรณ
"ในอดีตการผ่าตัดบายพาสหลอดเลือดหัวใจอาศัยประสบการณ์ของแพทย์เป็นส่วนใหญ่ ซึ่งบางครั้งอาจทำให้การรักษาไม่ได้ผลดีเท่าที่ควร ทำให้ผู้ป่วยต้องกลับมาเข้ารับการผ่าตัดใหม่ภายในเวลาไม่ถึง 1 ปี แต่หลังจากนี้เราสามารถใช้แบบจำลองคณิตศาสตร์ช่วยในการวางแผนการรักษาที่มีประสิทธิภาพกว่าเดิม แพทย์สามารถเห็นผลการรักษาก่อนการผ่าตัดจริงได้ และช่วยลดเวลาในการผ่าตัดบายพาสด้วย" ผศ.นพ.ทนงชัย กล่าว
ทั้งนี้ ทีมวิจัยใช้เวลาพัฒนาเทคนิคดังกล่าวมากว่า 2 ปี โดยได้รับทุนวิจัยของมหาวิทยาลัยมหิดล 1.3 ล้านบาท และได้ตีพิมพ์ผลวิจัยในวารสารนานาชาติหลายฉบับ เช่น ไบโอเมดิคัล ไซน์ (Biomedical Science) และ แมทเทอมาติคัล ไบโอไซน์ แอนด์ เอนจิเนียริง (Mathematical Bioscience and Engineering)
อย่างไรก็ตาม ขณะนี้การใช้เทคนิคการจำลองแบบคณิตศาสตร์เพื่อช่วยการผ่าตัดบายพาสดังกล่าวจะต้องใช้เวลาประมาณ 1 สัปดาห์ แพทย์จึงจะลงมือผ่าตัดได้ แต่ทีมวิจัยจะพัฒนาต่อไปอีกเพื่อให้ใช้เวลาลดลงเหลือ 2-3 ชั่วโมง หรือถ้าจะให้ดียิ่งขึ้นคือใช้เวลาเพียง 5-10 นาที เพราะผู้ป่วยบางรายอาจจำเป็นต้องได้รับการผ่าตัดอย่างเร่งด่วน.

Demand for interdisciplinary laboratories for physiology research by undergraduate students in biosciences and biomedical engineering

Abstract
Physiology as a discipline is uniquely positioned to engage undergraduate students in interdisciplinary research in response to the 2006–2011 National Science Foundation Strategic Plan call for innovative transformational research, which emphasizes multidisciplinary projects. To prepare undergraduates for careers that cross disciplinary boundaries, students need to practice interdisciplinary communication in academic programs that connect students in diverse disciplines. This report surveys policy documents relevant to this emphasis on interdisciplinary training and suggests a changing role for physiology courses in bioscience and engineering programs. A role for a physiology course is increasingly recommended for engineering programs,
but the study of physiology from an engineering perspective might differ from the study of physiology as a basic science. Indeed, physiology laboratory courses provide an arena where biomedical engineering and bioscience students can apply knowledge from both fields while cooperating in multidisciplinary teams under specified technical constraints. Because different problem-solving approaches are used by students of engineering and bioscience, instructional

The most visible contributions of biomedical engineering to clinical practice involve instrumentation for diagnosis, therapy, and rehabilitation. Cell and tissue engineering also have emerged as clinical realities. In the next 25 years, advances in electronics, optics, materials, and miniaturization will accelerate development of more sophisticated devices for diagnosis and
therapy, such as imaging and virtual surgery. The emerging new field of bioengineering—engineering based in the science of molecular cell biology—will greatly expand the scope of biomedical engineering to tackle challenges in molecular and genomic medicine.
